Who we are
“CoreVue”, “we”, “us” is a private specialist clinic at Ground Floor, 115 Cotham Road, Kew VIC 3101 operated by CoreVue Radiology (ABN 95 863 418 445) and CoreVue HeartCare (ABN 90 717 250 602). As a health service provider, we are bound by the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs) and the Health Records Act 2001 (Vic). This policy explains how we collect, use, store and disclose personal information, including health information.
What we collect
Depending on how you interact with us, we may collect:
- identity and contact details — name, date of birth, address, phone number and email address;
- health information — referral details, clinical notes, examination results, medical images and reports, Medicare number and other information needed to provide our services (this is “sensitive information” under the Privacy Act);
- referrer details — the name, provider number and practice details of referring practitioners;
- recruitment information — if you send us your CV or apply for a role;
- website usage data — see “Website analytics and cookies” below;
- payment and billing information, including health fund details where required to process claims;
- information provided by a parent, guardian, authorised representative or carer where appropriate.
How we collect it
We collect information directly from you — in person, by phone, by email or fax, or through the forms on this website — and from your referring doctor or other treating practitioners. Where appropriate, we may also receive information from hospitals, previous imaging providers, pathology services, Medicare, your private health insurer, or another person authorised to act on your behalf.
We only collect health information with your consent or as otherwise permitted by law. We collect only the personal information that is reasonably necessary to provide our services, operate our practice and comply with our legal obligations.
Where patients are children or lack capacity to make decisions, we may collect information from and communicate with a parent, guardian or other authorised representative in accordance with applicable law.
Why we collect it
- to provide medical imaging and cardiology services and manage your appointments;
- to report results to your referring doctor and, where relevant, other practitioners involved in your care;
- for billing, including Medicare and health fund claims;
- to respond to enquiries sent through our contact channels;
- for practice management, quality and safety activities;
- to consider job applications and expressions of interest;
- to comply with legal, regulatory and accreditation requirements;
- to conduct clinical audit, quality improvement and risk management activities.
Who we share it with
We may disclose your information to your referring doctor and other practitioners involved in your care, to Medicare and your health fund for billing purposes, and to contracted service providers who support our practice under confidentiality obligations. We do not sell personal information, and we do not routinely disclose personal information overseas. Otherwise, we only disclose personal information with your consent or where required or authorised by law.
We may also disclose information where reasonably necessary for accreditation, clinical audit, quality assurance, medical indemnity, legal proceedings, or to comply with a court order or other legal requirement.
Where applicable, imaging reports may also be made available through My Health Record in accordance with Australian law and your My Health Record privacy settings.
Where possible, we use de-identified information for quality improvement, service planning and statistical analysis so individuals cannot reasonably be identified.
Website analytics and cookies
This website uses Google Analytics to understand how visitors use the site (pages visited, approximate location and device type). This data is aggregated and does not identify you, and the contents of any forms you complete are never sent to the analytics service. You can opt out using Google's browser add-on.
Our website also uses cookies and similar technologies to enable core website functionality, improve user experience and monitor website performance. You can manage or disable cookies through your browser settings.
How we store and protect it
Your information is held in secure clinical and practice management systems with access limited to authorised staff. Information sent through this website is encrypted in transit (HTTPS). We retain health records for the periods required by Australian law.
We use physical, electronic and administrative safeguards including user authentication, audit logging, secure backups and access controls. We continuously monitor and update our systems to help protect against unauthorised access, cyber threats and other security risks.
Although we take reasonable steps to protect information transmitted over the internet, no method of electronic transmission or storage is completely secure.
Health records are retained for the minimum periods required under the Health Records Act 2001 (Vic) and other applicable legislation before being securely destroyed or de-identified where appropriate.
Recruitment information
If you apply for employment with CoreVue, we use the information you provide to assess your application, verify qualifications and experience, contact referees where appropriate and communicate with you about employment opportunities.
Access and correction
You may request access to the personal information we hold about you, and ask us to correct it if it is inaccurate, out of date or incomplete. Contact us using the details below; we will respond within a reasonable time. In limited circumstances access may be refused where the law allows. For your protection, we may require proof of identity before providing access to personal information or health records.
